Submit Reinstatement Documents
Follow all instructions provided in the Cover Letter issued with your order summary.
Key Points to Follow For NonProfit or General Business Entities:
- All documents must be submitted at the same time.
- The filer must request and receive the Certificate of Tax Clearance back from the Department of Revenue before submitting reinstatement documents.
- Documents should be completed and then scanned as separate PDFs in order to upload each to the portal.
- The only exception is the CORP50 AD and the Certificate of Tax Clearance must be a combined PDF.
- To upload, select choose file, then click the upload button to finalize. Only one PDF can be uploaded at a time.
- The upload for reinstatement should include the Application for Reinstatement (CORP 50AD), that was provided with the rescission packet request, and the DOR Certificate of Tax Clearance.
- Each completed registration report that is due should be uploaded separately. For each year required upload to the corresponding year.
To print a blank registration report online
- Click the link for Print Registration Report
- On the second Charter Number box enter the charter number or search by name of the entity for which you are filing a registration report
- Click print Blank Annual Report
- Make sure the entity details match your entity, the report type is your chosen type and the click Print Blank Annual Report

Tips for online filing:
Filing online will save time.
When creating an online account you will be sent a temporary password to use for the first time. You will be directed to update the password, you must do this to log in.
All addresses must be valid whether filing online or paper. Addresses can be checked by going to the United States Postal Service Address Lookup. If your address is not found please contact your local post office to have it added into the Address Management System.
Each entity is required to list their officers and directors. The filer must enter an individual more than once if they hold multiple roles.
If changing the registered agent, a letter of consent is required. The letter of consent is a statement from the new Registered Agent that has been signed and stating their consent to being the Registered Agent.
Note if you change the agent's address and the agent is a corporation the filing will be returned until the corporation has updated the address.
Our new system’s search is quite particular regarding spacing and punctuation. The least amount of information you can input during a search is best. If looking up an entity it is best to use the Charter Number found on the front of the reminder.
To retrieve and download a filed report choose My Account & Filing History on the lower right corner then select Filing History. You must be logged in to your online account to access this information.
PLEASE NOTE: ALL foreign entities must complete a change of agent form when reinstating.
