How to install the electronic forms used by the Records Management Division

The Forms Management Division of the Office of Administration has created electronic versions of the forms the Records Management Division uses. The installation of the forms is a two step procedure. First, the software that generates the form must be installed then the actual forms must be installed. Once the software and the forms are installed, you can fill out the forms on your computer.

There are two important things to remember. The Records Transmittal and Receipt form must be printed on carbon-less NCR paper, which is available from various approved vendors. The Box Labels must be printed on self-adhesive labels or glued on the box with a glue stick.


Installing the forms:

  1. On the Internet go to the following site, http://www.oa.mo.gov/gs/form/fm_eforms.htm, and print out the page. It has instructions on installing the software and forms.
  2. Click on the Operating System your computer uses (Windows 95, 98, NT).
  3. Follow the instructions on the page you printed to install the software. Once you are finished, you may have to restart your computer.
  4. Go back to the page linked in #1 above.
  5. Scroll down and click on the "Forms for Windows 95, 98 or NT" link. Scroll down and click on "All Forms in Records Management Group – Universal" and follow the instructions on the page you printed out.
