| SUBJECT: | Certification and Reporting Requirements |
Missouri public libraries are subject to six (6) certification and reporting requirements. The required certification set, State Aid application for public libraries and public library Board of Trustees forms, is due June 30th of the current calendar year. These forms are generally available online by mid-May at http://www.sos.mo.gov/library/development; then click on the "Public Library Certifications" link in the left side menu.
Note the following certification and report forms listed by due date:
April 11, 2011
1. Technology Plan Update
Due to FCC changes to the E-rate program required in the 6th Report and Order, a written Technology Plan is no longer required for E-rate funding for all Priority 1 services, which include Internet Connectivity and Basic Telephone Service. If a library only plans to file for E-rate funding for Priority 1 services (including allowing MOREnet to file for the library's Internet Access on the library's behalf), the library is not required to write or seek approval of a Technology Plan specifically for E-rate.
A library is required to write a Technology Plan and seek approval from the State Library if the library plans to file for Priority 2 funding. Priority 2 includes funding for Internal Connections or Basic Maintenance of Internal Connections, a Technology Plan and annual Technology Plan Update forms are still required and should be submitted to the State Library for reading and approval in the spring before the current Technology Plan expires.
2. Certification of Compliance
The public library certification of compliance with the RSMo 182.255 and 182.287 is also due. Indicate on the form the filter service used by your library. Those libraries choosing not to filter may still comply with Missouri statutes IF they have an approved policy for protecting minors from pornographic materials.

3. State Aid Application form and 4. Board of Trustees list
Although the state aid application forms are available online, they must be printed off, completed with the required official and notarized signatures and submitted by mail. A list of each public library's board members must also be provided in addition to the state aid application form(s). The State Aid application and of Board Trustees list must be submitted on or before June 30, 2011.

5. Summer Reading Program Evaluation, early September 2011

6. Annual Public Library Survey, Fall 2011
Annual statistical data on public libraries are collected nationwide for submission to IMLS and in many states as a requirement of receiving State Aid. An online software program will again be used to expedite gathering these data as well as the timely provision of the statistical information back to you. All libraries must submit their data online. Exceptions to submit the survey by mail will be made for those libraries without Internet access. The availability of the online form will be announced in Show Me Express, our weekly electronic newsletter.
